本稿が問うのは、部位の間の 5 年生存率の違いが、「どの進行度で見つかる人が多いか」（進行度の構成）と「同じ進行度どうしの生存率の違い」（進行度ごとの生存率）にどう分かれるか、である。全国がん登録の 2018 年診断例の 5 年相対生存率（臨床進行度別・男女計）を使い、進行度の分かる人について、二つの効果に分けた。新しい数学定理も新しい法則も主張しない。

第一に、三つの進行度（限局・領域・遠隔）の生存率を三つの割合で重みづけた平均（本稿の計算・「進行度の分かる人の生存率」）は、甲状腺の 96.1 から膵臓の 13.6 まで開く。全部位は 68.2 である。進行度不明の人の生存率は表に無く、全体の値から逆算すると膵臓の 8.67 から前立腺の 113.89 まで開く。逆算が当たっているかは確かめられないので、本稿は不明の人を入れずに比べる（第3・4節）。

第二に、これが本稿の芯である。膀胱の生存率は、限局 81.0（肺 81.1）・領域 38.9（肺 39.3）・遠隔 11.0（肺 11.7）と、三つの進行度のどれでも肺とほぼ同じ（差は三つとも標準誤差の 2 倍の中）なのに、合わせると 69.6 対 41.9 になる。 差 +27.7 点のうち +28.0 点が進行度の構成の効果で、進行度ごとの効果は −0.3 点である。膀胱は限局で見つかる人が 66.8 %、肺は 31.3 % で、遠隔は 5.3 % と 38.8 % である（第5節）。

第三に、19 部位の 171 組のうち 10 組で、三つの進行度のどれでも生存率が低い部位のほうが、合わせると高くなる（膀胱と肺・喉頭と結腸・皮膚と乳房など）。これは数理物理の論文183 が「同じ表が、二つの正しい答を持つ」として示した形（シンプソンの逆転）の、がんの生存率の実例である（第5節）。

第四に、全部位（68.2）との差を分けると、構成の効果のほうが大きい部位は 19 部位のうち 8 部位（膀胱・喉頭・皮膚・子宮体部など）で、残りは進行度ごとの効果のほうが大きい。膵臓は差 −54.5 点のうち、進行度ごとの効果が −36.6 点、構成の効果が −17.9 点で、限局でも 42.4 %（全部位の限局は 91.2 %）である（第6節）。

第五に、19 部位の差のばらつき（分散 515.5）は、構成の効果の分散 94.7、進行度ごとの効果の分散 275.7、二つの共分散の 2 倍 145.1 に分かれる。二つの効果は、12 部位で同じ向き、7 部位で逆向きである（第6節）。

第六に、分けているのは、進行度の構成と、進行度ごとの生存率である。部位の 5 年生存率の高さは、そのがんの性質（進みやすさ・治りやすさ）だけでなく、どの進行度で見つかる人が多いかも運んでいる。

Abstract

This paper asks how differences in 5-year survival between cancer sites split into “at which stage more people are found” (the stage mix) and “differences in survival within the same stage” (stage-specific survival). Using the National Cancer Registry’s 5-year relative survival for cases diagnosed in 2018 (by clinical stage, both sexes), it splits the differences for people with known stage into these two effects. It claims no new mathematical theorem and no new law.

First, the average of survival at the three stages (localized, regional, distant) weighted by the three shares (this paper’s calculation, “survival of people with known stage”) runs from 96.1 for thyroid to 13.6 for pancreas; all sites is 68.2. Survival of people with unknown stage is not in the table; back-calculated from the overall figure it runs from 8.67 for pancreas to 113.89 for prostate. Since whether that back-calculation is right cannot be checked, the paper compares without the unknown-stage group (Sections 3 and 4).

Second, this is the core of the paper. Bladder survival is localized 81.0 (lung 81.1), regional 38.9 (lung 39.3), and distant 11.0 (lung 11.7)（nearly the same as lung at every one of the three stages (all three differences within two standard errors)）yet combined it is 69.6 versus 41.9. Of the +27.7-point difference, +28.0 points is the stage-mix effect and the stage-specific effect is −0.3 points. 66.8% of bladder cases are found localized against 31.3% for lung, and 5.3% versus 38.8% distant (Section 5).

Third, in 10 of the 171 pairs of 19 sites, the site with lower survival at every one of the three stages is higher when combined (bladder and lung, larynx and colon, skin and breast, and others). This is a cancer-survival example of the pattern (Simpson’s reversal) that Paper 183 of the mathematical-physics series showed as “One Table, Two Correct Answers” (Section 5).

Fourth, splitting the difference from all sites (68.2), the stage-mix effect is the larger one for 8 of the 19 sites (bladder, larynx, skin, corpus uteri, and others), and the stage-specific effect is larger for the rest. For pancreas, of a −54.5-point difference, the stage-specific effect is −36.6 points and the stage-mix effect −17.9 points; even localized it is 42.4% (all sites localized: 91.2%) (Section 6).

Fifth, the spread of the differences over 19 sites (variance 515.5) splits into the variance of the stage-mix effect 94.7, the variance of the stage-specific effect 275.7, and twice their covariance 145.1. The two effects point the same way at 12 sites and opposite ways at 7 (Section 6).

Sixth, what separates them is the stage mix and the survival within each stage. A site’s 5-year survival carries not only the nature of that cancer (how fast it progresses, how treatable it is) but also at which stage more people are found.
