要旨
令和 7 年（2025 年）の国民生活基礎調査 介護票の二つの表（「いまの要介護度 × 一年前の要介護度」（第28表）と「この一年間の要介護度の変化」（第29表））で、要介護度が一年でどう動いたかを、一年前の側といまの側の二つから数えて並べた。新しい数学定理も新しい法則も主張しない。
第一に、表はそろっている。列の和・行の和・男 ＋ 女 と総数の食い違いは、どれも 10 万対で 2 以下（丸めの幅）である（第3節）。
第二に、第29表の「重くなった 29,556・変化なし 60,007・軽くなった 6,471・不詳 3,966」は、第28表を畳んだ数と 6 以内で一致する。 「変化なし」と「軽くなった」は七つの段（要支援 1〜要介護 5）の中の動きそのものだが、「重くなった」には、段が上がった 16,586 のほかに、一年前は自立と認定されていたか認定を申請していなかった 12,972 が入っている。重くなった分のうち 0.439 は、一年前に認定を受けていなかった人である（第4節）。
第三に、そのため「重くなった ÷ 軽くなった」は、第29表のままでは 4.57、一年前も認定を受けていた人だけでは 2.56 になる。どの年齢でも重くなった割合は軽くなった割合の 2 倍を超え、重くなった割合がいちばん大きいのは 65〜69 歳（0.392）である（第4節・第5節）。
第四に、ここが本稿の芯である。一年前の段ごとに数えると、段が上がった割合は要支援 1 の 0.254 から要介護 4 の 0.085 まで小さくなり、段が下がった割合は要介護 3 で 0.176、要介護 4 で 0.231 と大きくなって、要介護 3・4・5 では下がった割合が上がった割合を上回る（第6節）。
第五に、同じ段にとどまった割合は、どの段でも、一年前の側から数えるほうがいまの側から数えるより大きい。 差がいちばん大きいのは要介護 5 で、一年前の側から 0.904、いまの側から 0.629。いまの要支援 1 のうち 0.232 は、一年前に認定を受けていなかった（第7節・第8節）。
第六に、分けるのは一年前の側から数えるか、いまの側から数えるか、一年前に認定を受けていたか。表に残るのは、調査の月にいまも世帯で介護を要する人だけで、この一年に亡くなった人や施設に移った人は入っていない。
Abstract
Using two tables from the long-term care questionnaire of the 2025 Comprehensive Survey of Living Conditions（"level of care needed now × level one year ago" (Table 28) and "change in the level of care needed over the past year" (Table 29)）how care levels moved in one year was counted from two sides, from one year ago and from now, and lined up. It claims no new mathematical theorem and no new law.
First, the tables are consistent. The discrepancies in column sums, row sums, and men + women versus total are all 2 or less per 100,000 (the rounding width) (Section 3).
Second, Table 29's "became heavier 29,556, no change 60,007, became lighter 6,471, unknown 3,966" matches the collapsed Table 28 to within 6. "No change" and "became lighter" are exactly the movements within the seven levels (Support Level 1 to Care Level 5), but "became heavier" contains, besides the 16,586 who moved up a level, 12,972 who a year ago had been assessed as independent or had not applied for certification. Of those who became heavier, 0.439 were not certified a year ago (Section 4).
Third, therefore, "heavier ÷ lighter" is 4.57 in Table 29 as is, but 2.56 among those also certified a year ago. At every age the share becoming heavier is more than twice the share becoming lighter, and the share becoming heavier is largest at 65–69 (0.392) (Sections 4 and 5).
Fourth, this is the core of the paper. Counted by the level one year ago, the share moving up falls from 0.254 at Support Level 1 to 0.085 at Care Level 4, the share moving down grows to 0.176 at Care Level 3 and 0.231 at Care Level 4, and at Care Levels 3, 4, and 5 the share moving down exceeds the share moving up (Section 6).
Fifth, at every level, the share staying at the same level is larger counted from one year ago than counted from now. The gap is largest at Care Level 5: 0.904 from one year ago and 0.629 from now. Of those now at Support Level 1, 0.232 were not certified a year ago (Sections 7 and 8).
Sixth, what separates them is whether one counts from one year ago or from now, and whether the person was certified a year ago. The table keeps only those who, in the survey month, still need care and live in a household; people who died or moved into a facility during the year are not in it.
