要旨

臨床試験の登録簿 ClinicalTrials.gov（U.S. National Library of Medicine）の API で、登録全体と、がんの 18 部位（M113 と同じ並び）の試験の数を、種類・段階・状況ごとに数えた（2026 年 9 月 27 日取得・ClinicalTrials.gov がデータを処理した日は 2026-09-25T09:00:04）。試験の良し悪しや、どの病気の試験が多い・少ないの良し悪しは言わない。新しい数学的定理も新しい法則も主張しない。

第一に、これが本稿の芯である。登録全体の介入試験は 461,397 件で、段階の札（早期第 1 相・第 1 相・第 2 相・第 3 相・第 4 相・段階なし）を数えて足すと 485,973 枚、介入試験の 1.0533 倍になった。第 1 相の札 65,822 枚のうち 0.260 は第 2 相の札も持っていた（第 5 節）。

第二に、登録する人が段階を選ぶのは一つだけ（"Select only one."）だが、「第 1 相／第 2 相」「第 2 相／第 3 相」を選ぶと、API では二枚の札になる。介入試験 ＝ 札の和 − 第 1 相かつ第 2 相（17,095）− 第 2 相かつ第 3 相（7,606）＋ 段階が空（125）が、登録全体と 18 部位の 19 通りすべてでちょうど成り立った（第 3・5 節）。

第三に、がんの 18 部位では、札の和は介入試験の 1.0778 倍（乳房）〜1.1585 倍（白血病）で、18 部位すべてが登録全体より大きかった。段階なし（機器や行動の試験など）の割合が 0.113〜0.376 と登録全体（0.515）より小さく、薬の段階を持つ試験の中でも二つの札を持つ割合が 0.1236〜0.1876 と登録全体（0.1104）より大きかった（第 5 節）。

第四に、登録の数と、いま参加を募っている数は別である。膵臓の登録 5,089 件のうち募集中は 896 件（0.176）、募集中の介入試験は 679 件、そのうち日本に実施場所があるのは 40 件だった（第 6 節）。

第五に、状況不明（完了予定日を過ぎても 2 年以上状況が確かめられていない試験で、ClinicalTrials.gov は閉じた試験として扱う）は、登録全体で 96,838 件と、募集中 65,044 件の 1.489 倍あった。がんの 18 部位のうち 11 部位で、状況不明が募集中より多かった（第 6 節）。

第六に、分けるのは 試験で数えるか、段階の札で数えるか である。「第 1 相の試験は何件」と聞いたら、第 1 相／第 2 相の試験が入っているかを確かめる。「この病気の試験は何件」と聞いたら、そのうちいま募集しているのは何件かを合わせて確かめる。

Abstract

Using the API of the clinical trial registry ClinicalTrials.gov (U.S. National Library of Medicine), this paper counted studies by type, phase, and status for all registered studies and for 18 cancer sites (the same list as M113) (retrieved 27 September 2026; the data were processed by ClinicalTrials.gov on 2026-09-25T09:00:04). Whether trials are good or bad, or whether a disease has more or fewer trials, is not discussed. No new mathematical theorem and no new law are claimed.

First, and this is the core of this paper, there were 461,397 interventional studies in all, and counting their phase labels (Early Phase 1, Phase 1, Phase 2, Phase 3, Phase 4, and not applicable) and adding them up gave 485,973 labels, 1.0533 times the interventional studies. Of the 65,822 Phase 1 labels, 0.260 belonged to studies that also carry a Phase 2 label (Section 5).

Second, a registrant selects only one phase ("Select only one."), but choosing "Phase 1/Phase 2" or "Phase 2/Phase 3" gives two labels in the API. Interventional studies = sum of labels − Phase 1 and Phase 2 (17,095) − Phase 2 and Phase 3 (7,606) + no phase (125) held exactly in all 19 cases (all registered studies and 18 sites) (Sections 3 and 5).

Third, for the 18 cancer sites, the sum of labels was 1.0778 (breast) to 1.1585 (leukemia) times the interventional studies, larger than for all registered studies at every site. The share without phases (such as device or behavioral studies) was 0.113–0.376, smaller than for all studies (0.515), and even among studies with a drug phase, the share with two labels was 0.1236–0.1876, larger than for all studies (0.1104) (Section 5).

Fourth, the number registered and the number seeking participants now are different. Of the 5,089 registered pancreas studies, 896 (0.176) were recruiting, 679 were recruiting interventional studies, and 40 of those had a location in Japan (Section 6).

Fifth, studies with unknown status (studies whose status has not been verified for more than 2 years after their completion date, which ClinicalTrials.gov treats as closed) numbered 96,838 in all, 1.489 times the 65,044 recruiting studies. At 11 of the 18 cancer sites, unknown status outnumbered recruiting (Section 6).

Sixth, what separates them is whether you count studies or phase labels. When you hear "there are so many Phase 1 trials," check whether Phase 1/Phase 2 trials are included. When you hear "there are so many trials for this disease," also check how many of them are recruiting now.
