要旨

厚生労働省の国民医療費（2023 年度・2022 年度の性・年齢階級別の表と、2000〜2021 年度の年次・年齢階級別の表）で、一人当たりの医療費が年齢とともにどう増えるか、2000→2023 年度の総額の伸びが年齢構成と一人当たりのどちらから来たかを見た。新しい数学定理も新しい法則も主張しない。

第一に、表はそろっている。年齢階級の和は総数に、医科は入院と入院外の和に、男女の和は総数に、丸めの幅で一致し、一人当たりは医療費を人口で割った値に一致する。2021〜2023 年度の総数は第1表と一致する（第3節）。

第二に、ここが本稿の芯である。2023 年度の一人当たり国民医療費は、40〜84 歳で 16.52 年ごとに倍になる（決定係数 0.9982）。同じ窓で M002 が測った死亡率の倍になる年数（2024 年・男 6.99 年・女 7.60 年）と比べると、医療費は男で 2.054 倍、女で 2.522 倍の年数がかかる。 一人当たりがいちばん低いのは 20〜24 歳（106.2 千円）で、0〜4 歳はその 2.739 倍、90 歳以上は 11.269 倍である（第4節）。

第三に、種類で形が違う。倍になる年数は、入院 11.71 年・入院外 19.67 年・薬局調剤 20.17 年・歯科 45.84 年で、入院だけが 90 歳以上まで上がり続け、入院外と歯科は 80〜84 歳、薬局調剤は 85〜89 歳で山になる（第5節）。

第四に、2000→2023 年度に総額は 301,416 から 480,913 億円（1.596 倍）になった。2000 年度の年齢ごとの一人当たりのまま年齢構成だけを 2023 年度にすると 1.337 倍、2000 年度の年齢構成のまま一人当たりだけを 2023 年度にすると 1.274 倍、人口の大きさだけなら 0.980 倍である。 65 歳以上の人口の割合は 0.174 から 0.291 になった（第6節）。

第五に、年齢ごとの一人当たりの伸びは、0〜14 歳の 1.738 倍から 70〜74 歳の 1.051 倍まで、年齢が上がるほど小さい（75 歳以上は 1.138 倍）。 高齢の群の医療費が増えた分は、おもに人数の増え（75 歳以上 2.228 倍）から来ている（第6節）。

第六に、分けるのは年齢とともに増える速さ（倍になる年数）と、人がどの年齢にいるか（年齢構成）である。「医療費は年を取るほどかかる」は倍になる年数の話で、その速さは死亡率の半分に届かない。「医療費が増えた」は年齢構成と一人当たりの両方の話で、どちらが大きかったかは年齢の群ごとに違う。

Abstract

Using the Ministry of Health, Labour and Welfare's National Medical Expenditure (tables by sex and age group for FY2023 and FY2022, and the table by year and age group for FY2000–FY2021), this paper looked at how per-capita medical expenditure rises with age, and whether the growth of the total from FY2000 to FY2023 came from the age composition or from per-capita expenditure. No new mathematical theorem and no new law are claimed.

First, the tables are consistent. Age groups sum to the total, medical care equals inpatient plus outpatient, men plus women equal the total, all within rounding, and per-capita values equal expenditure divided by population. The totals for FY2021–FY2023 match Table 1 (Section 3).

Second, this is the core of the paper. In FY2023, per-capita national medical expenditure doubles every 16.52 years at ages 40–84 (coefficient of determination 0.9982). Compared with the doubling time of death rates that M002 measured in the same window (2024, men 6.99 years, women 7.60 years), medical expenditure takes 2.054 times as many years for men and 2.522 times for women. Per-capita expenditure is lowest at 20–24 (106.2 thousand yen); ages 0–4 are 2.739 times that and 90+ are 11.269 times (Section 4).

Third, the shape differs by type. The doubling times are 11.71 years for inpatient care, 19.67 for outpatient care, 20.17 for pharmacy dispensing, and 45.84 for dental care; only inpatient care keeps rising up to 90+, while outpatient and dental care peak at 80–84 and pharmacy dispensing at 85–89 (Section 5).

Fourth, from FY2000 to FY2023 the total went from 301,416 to 480,913 hundred million yen (1.596-fold). Keeping FY2000 per-capita expenditure by age and moving only the age composition to FY2023 gives 1.337-fold; keeping the FY2000 age composition and moving only per-capita expenditure gives 1.274-fold; population size alone gives 0.980-fold. The share of the population aged 65+ went from 0.174 to 0.291 (Section 6).

Fifth, growth in per-capita expenditure by age shrinks with age, from 1.738-fold at 0–14 to 1.051-fold at 70–74 (1.138-fold at 75+). The increase for the oldest groups came mainly from growth in their numbers (75+: 2.228-fold) (Section 6).

Sixth, what separates them is the speed of increase with age (the doubling time) versus where people are in age (the age composition). "Medical costs rise as people age" is about the doubling time, and that speed is less than half that of death rates. "Medical costs have risen" is about both the age composition and per-capita expenditure, and which was larger differs by age group.
