厚生労働省の薬価基準収載品目リスト（令和８年８月13日適用・12,428 品目）と、社会医療診療行為別統計（令和7年8月審査分）の薬局 第4表（薬剤点数を薬価の階級で分けた表）で、薬価の分布がどれだけ偏っているか、品目の数で見る偏りと使われた金額で見る偏りがどう違うかを見た。品名・製造販売業者の名前・成分名は一つも出さない。新しい数学定理も新しい法則も主張しない。

第一に、表はそろっている。リストの品目数の合計は 12,428 で、ページの「約1万3千程度」の幅に入る。第4表の階級の和は、内服薬で薬剤料逓減の分だけ総数より大きく、注射薬と外用薬では総数に一致する（第3節）。

第二に、ここが本稿の芯である。内用薬 7,064 品目の薬価の中央値は 20.1 円で、500 円以上の品目は 0.0810 しかない。ところが薬局の内服薬の薬剤点数では、500 円以上の薬が 0.3064 を占める。50 円未満の品目は 0.7380 で、点数では 0.3103 である（第5節）。

第三に、薬価の分布は、どの区分でも対数で見ないと形が分からないほど広い。90 % 点 ÷ 10 % 点は、内用薬 42.8・外用薬 172.6・注射薬 836.1 である。 算術平均は中央値の、内用薬で 89.18 倍、注射薬で 249.81 倍になる（第4節）。

第四に、注射薬は、500 円以上の品目が 0.6821 で、薬局の注射薬の点数の 0.9975 を占める。 外用薬は 50 円未満の品目が 0.6032 で点数の 0.3053、500 円以上が品目の 0.1505 で点数の 0.3508 である（第5節）。

第五に、中央値は内用薬 20.1 円 ＜ 外用薬 27.6 円 ＜ 注射薬 1,398.0 円の順である（第4節）。

第六に、分けるのは品目の数で数えるか、使われた金額で数えるかである。「安い薬がほとんど」は品目の数の話で、使われた金額で見れば、内服薬の点数は 50 円未満と 500 円以上がほぼ同じ大きさ（0.3103 と 0.3064）に分かれる。

Abstract

Using the Ministry of Health, Labour and Welfare's list of items in the National Health Insurance drug price standard (as of 13 August 2026, 12,428 items) and Pharmacy Table 4 of the Statistics of Medical Care Activities in Public Health Insurance (claims reviewed in August 2025)（drug points split by price class）this paper looked at how skewed the distribution of drug prices is, and how the skew by number of items differs from the skew by money spent. No product names, manufacturer names, or ingredient names are given. No new mathematical theorem and no new law are claimed.

First, the tables are consistent. The list's items total 12,428, within the page's "about 13 thousand." In Table 4, the sum of price classes exceeds the total for oral drugs by the drug-cost reduction, and equals the total for injections and external drugs (Section 3).

Second, this is the core of the paper. The median price of the 7,064 oral items is 20.1 yen, and items at 500 yen or more are only 0.0810 of them. Yet in pharmacy oral-drug points, drugs at 500 yen or more take 0.3064. Items under 50 yen are 0.7380, and in points 0.3103 (Section 5).

Third, drug prices are spread so widely that their shape shows only on a log scale. The 90th percentile ÷ 10th percentile is 42.8 for oral drugs, 172.6 for external drugs, and 836.1 for injections. The arithmetic mean is 89.18 times the median for oral drugs and 249.81 times for injections (Section 4).

Fourth, for injections, items at 500 yen or more are 0.6821 and take 0.9975 of pharmacy injection points. For external drugs, items under 50 yen are 0.6032 and take 0.3053 of points, and items at 500 yen or more are 0.1505 and take 0.3508 (Section 5).

Fifth, the medians run oral 20.1 yen < external 27.6 yen < injection 1,398.0 yen (Section 4).

Sixth, what separates them is whether one counts by number of items or by money spent. "Most drugs are cheap" is about the number of items; by money spent, oral-drug points split into nearly equal parts under 50 yen and at 500 yen or more (0.3103 and 0.3064).
