要旨
令和7年国民生活基礎調査（介護票）で、介護が必要になった原因を、主な原因（一つ）と原因（複数回答）の二つの表で、性と年齢ごとに並べた。数は「介護を要する者 10 万対」（介護を要する者の総数を 100,000 としたときの数）である。新しい数学定理も新しい法則も主張しない。
第一に、表はそろっている。主な原因の和・男女の和・年齢の和は丸めの幅（最大 3）で総数に一致し、複数回答の和はどの升目でも総数以上、要介護度別の表の原因ごとの総数は主な原因の表と一致する（第3節）。
第二に、男女計の 1 位は、主な原因では認知症（16,743）、複数回答では高齢による衰弱（26,100）である。 男性は主な原因でも複数回答でも脳血管疾患が 1 位（6,737・8,997）。女性の主な原因は、高齢による衰弱 11,821・骨折・転倒 11,739・認知症 11,723 と、1 位と 3 位の差が 98（女性の計の 0.0015）しかない（第4節）。
第三に、ここが本稿の芯である。主な原因の 1 位は年齢で入れ替わる。 男女計では 65〜79 歳が脳血管疾患（75〜79 歳で 0.231）、80〜89 歳が認知症（85〜89 歳で 0.224）、90 歳以上が高齢による衰弱（0.298）。高齢による衰弱の割合は 65〜69 歳の 0.019 から 90 歳以上の 0.298 まで年齢とともに上がり、脳血管疾患は 75〜79 歳から下がる。複数回答でも、入れ替わりの順は同じである（第5節）。
第四に、複数回答 ÷ 主な原因は、原因によって 1.25 から 3.50 まで違う。 視覚・聴覚障害 3.50・糖尿病 3.12・呼吸器疾患 2.61・心疾患 2.53 の四つは、挙がることは多いが主な原因にはなりにくい。パーキンソン病 1.25・脳血管疾患 1.33・認知症 1.35 は、挙がればほぼ主な原因になる（第6節）。
第五に、主な原因によって要介護度の構成も違う。 主な原因が認知症の人は要支援の割合が 0.126 と小さく、関節疾患は 0.600、視覚・聴覚障害は 0.592 と大きい（第7節）。
第六に、分けるのは原因を一つだけ選ぶか、当てはまるものを全部選ぶか。「介護が必要になった原因の 1 位」は、選び方と年齢と性によって答えが替わる。
Abstract
Using the 2025 Comprehensive Survey of Living Conditions (long-term care questionnaire), the causes for needing care were lined up by sex and age in two tables: the main cause (one) and causes (multiple answers). The numbers are "per 100,000 people needing care" (the total number of people needing care set to 100,000). It claims no new mathematical theorem and no new law.
First, the tables are consistent. The sums of main causes, sexes, and ages match the totals within the rounding width (at most 3), the sum of multiple answers is at least the total in every cell, and the total per cause in the table by care level agrees with the main-cause table (Section 3).
Second, for both sexes the leading cause is dementia (16,743) as the main cause, and frailty due to old age (26,100) in multiple answers. For men, cerebrovascular disease leads in both (6,737 and 8,997). For women, the main causes are frailty 11,821, fracture or fall 11,739, and dementia 11,723, with only 98 between first and third (0.0015 of the women's total) (Section 4).
Third, this is the core of the paper. The leading main cause shifts with age. For both sexes, it is cerebrovascular disease at 65–79 (0.231 at 75–79), dementia at 80–89 (0.224 at 85–89), and frailty due to old age at 90+ (0.298). The share of frailty rises with age from 0.019 at 65–69 to 0.298 at 90+, and cerebrovascular disease falls from 75–79. In multiple answers, the order of the shift is the same (Section 5).
Fourth, multiple answers ÷ main cause ranges from 1.25 to 3.50 by cause. Visual or hearing impairment 3.50, diabetes 3.12, respiratory disease 2.61, and heart disease 2.53 are often named but rarely become the main cause. Parkinson's disease 1.25, cerebrovascular disease 1.33, and dementia 1.35 are almost always the main cause when named (Section 6).
Fifth, the composition by care level also differs by main cause. Those whose main cause is dementia have a small share at the support levels, 0.126, while joint disease has 0.600 and visual or hearing impairment 0.592 (Section 7).
Sixth, what separates them is whether one cause is chosen or all that apply. "The leading cause for needing care" changes with how causes are chosen, with age, and with sex.
