要旨

人口動態統計の死因簡単分類（1995〜2024 年の年次と、2005・2016・2024 年の 5 歳階級）で、血管性及び詳細不明の認知症とアルツハイマー病の死亡の増え方を、人口の年齢構成・年齢ごとの率・2017 年の数え方の変更の三つに分けて並べた。新しい数学定理も新しい法則も主張しない。

第一に、表はそろっている。どの年も 総数 ＝ 男 ＋ 女、5 歳階級の和 ＋ 不詳 ＝ 総数 で、2005・2016・2024 年の下巻 第2表の総数は、同じ年の上巻 第5.13表の数と一致する（第3節）。

第二に、死亡数は 1995→2024 年に、血管性及び詳細不明の認知症で 2,697 から 24,666（9.1 倍）、アルツハイマー病で 511 から 25,595（50.1 倍）になった。 2014→2024 年の隣り合う年の比でいちばん大きいのは、どちらも 2016→2017 年（1.643・1.440）で、ほかの年は 0.973〜1.090、1.006〜1.135 に収まる（第4節）。

第三に、ここが本稿の芯である。40 歳以上のアルツハイマー病の死亡は 2005→2024 年に 14.12 倍になり、率を 2005 年のままにして人口の年齢構成だけを 2024 年にすると 1.93 倍、人口を 2005 年のままにして年齢ごとの率だけを 2024 年にすると 6.65 倍になる。 血管性及び詳細不明の認知症は 7.40 倍が 2.28 倍と 3.42 倍に分かれる。二つの死因・三つの期間のどれでも、率の分のほうが年齢構成の分より大きい（第5節）。

第四に、2005→2016 年のアルツハイマー病の率の倍率は、80〜84 歳の 3.38 から 100 歳以上の 9.48 まで、年齢が高いほど大きい。 血管性及び詳細不明の認知症は、同じ年齢の幅で 2.01〜2.29 倍に収まる（第6節）。

第五に、2016→2017 年の比を、同じ死亡票を新旧の分類で数え直した比（M041 の新旧比）で割ると、血管性及び詳細不明の認知症は 1.643 ÷ 1.509 ＝ 1.089、アルツハイマー病は 1.440 ÷ 1.296 ＝ 1.111 になり、どちらもほかの年の比の幅に入る。 新旧比が年齢によらず同じに効いたと置くと、2016→2024 年の率の分は、血管性及び詳細不明の認知症で 1.05、アルツハイマー病で 1.27 になる（第7節）。

第六に、分けるのは人口の年齢構成か、年齢ごとの率か、2017 年の数え方の変更か。認知症の死亡の増え方には三つとも入っていて、二つの死因で配分が違う。

Abstract

Using the condensed list of causes of death in the Vital Statistics (yearly for 1995–2024, and by 5-year age group for 2005, 2016, and 2024), the rise in deaths from vascular and unspecified dementia and from Alzheimer's disease was divided into three parts and lined up: the age composition of the population, the age-specific rates, and the 2017 change in how deaths are counted. It claims no new mathematical theorem and no new law.

First, the tables are consistent. In every year, total = men + women and the sum of 5-year groups + unknown = total, and the totals in Volume 2 Table 2 for 2005, 2016, and 2024 match the figures for the same year in Volume 1 Table 5.13 (Section 3).

Second, from 1995 to 2024, deaths from vascular and unspecified dementia went from 2,697 to 24,666 (9.1-fold), and deaths from Alzheimer's disease from 511 to 25,595 (50.1-fold). Among the year-to-year ratios for 2014–2024, the largest for both is 2016 to 2017 (1.643 and 1.440); the other years lie within 0.973–1.090 and 1.006–1.135 (Section 4).

Third, this is the core of the paper. Deaths from Alzheimer's disease at ages 40+ rose 14.12-fold from 2005 to 2024; keeping the rates at 2005 and moving only the age composition to 2024 gives 1.93-fold, and keeping the population at 2005 and moving only the age-specific rates to 2024 gives 6.65-fold. For vascular and unspecified dementia, 7.40-fold divides into 2.28-fold and 3.42-fold. For both causes and all three periods, the rate part is larger than the age-composition part (Section 5).

Fourth, from 2005 to 2016, the ratio of Alzheimer's disease rates grows with age, from 3.38 at 80–84 to 9.48 at 100+. For vascular and unspecified dementia it stays within 2.01–2.29 over the same ages (Section 6).

Fifth, dividing the 2016-to-2017 ratio by the ratio from recoding the same death records under the new and old classifications (the new/old ratio of M041) gives 1.643 ÷ 1.509 = 1.089 for vascular and unspecified dementia and 1.440 ÷ 1.296 = 1.111 for Alzheimer's disease, both within the range of the other years. Assuming the new/old ratio acted equally at all ages, the rate part from 2016 to 2024 becomes 1.05 for vascular and unspecified dementia and 1.27 for Alzheimer's disease (Section 7).

Sixth, what separates them is the age composition of the population, the age-specific rates, or the 2017 change in how deaths are counted. The rise in dementia deaths contains all three, and the two causes divide them differently.
