要旨

本稿が問うのは、がんの「5 年生存率」が、診断から何年か過ごした人にとって同じ意味を持つか、である。答えは、持たない、である。診断から 1 年、2 年…と過ごした人の「その先の 5 年」の生存率は、進んだ病期ほど大きく上がり、病期のあいだの差は縮む。そしてその形は、一人ひとりの体が変わったことでも、早く亡くなる人が集団から抜けたことでも作れるので、集計の数からはどちらかを決められない。新しい数学定理も新しい法則も主張しない。

第一に、ふつうに言う「5 年生存率」は、診断した日から 5 年の生存率である。院内がん登録の 2012 年診断例の集計は、同じ人たちについて、診断から t 年（t＝0〜5）生きた人の「その先の 5 年」の実測生存率（サバイバー生存率）を、10 部位 × 病期Ⅰ〜Ⅳについて示している。本稿はその 40 本をすべて使う（第3・4節）。

第二に、これが本稿の芯である。Ⅳ期では 10 部位すべてで、5 年生きた人の次の 5 年（t＝5）が、診断時の 5 年生存率（t＝0）を上回る。差の中央値は +42.2 点である。 膵臓がんではⅠ期 44.0 % → 68.7 %、Ⅱ期 18.3 % → 59.0 %、Ⅲ期 4.7 % → 50.1 %、Ⅳ期 1.3 % → 42.5 %、胃がんⅣ期は 5.5 % → 61.2 % である（第5節）。

第三に、病期のあいだの幅（最も高い病期と最も低い病期の差）は、10 部位すべてで t＝5 のほうが小さい。 幅の中央値は t＝0 の 55.5 点から t＝5 の 22.4 点になる（第5節）。

第四に、40 本のうち 8 本は t＝5 のほうが低い（前立腺がんⅠ〜Ⅲ期 −6.6〜−7.1 点、胃がんⅠ期 −3.4 点、乳がんⅠ・Ⅱ期 −2.4・−1.8 点など）。8 本ともⅠ〜Ⅲ期である。この値は実測生存率なので、がん以外の死亡も含む（第6節）。

第五に、上がる形は二つの別の仕組みの模型のどちらでも作れる。誰の危険も時間で変わらない「二群」の模型（危険の高い群が先に抜けていくだけ）は、上がる 32 本に中央値 0.63 点の外れで当てはまり、全員の危険が時間とともに下がる「一群」の模型も中央値 1.38 点で当てはまる。どちらの模型も誰の危険も上がらない形なので、下がる 8 本は作れない（第7節）。

第六に、数には幅がある。Ⅳ期の膵臓がんで 5 年時点に生きていた人は、集計対象 6,242 人の 1.3 % で、概算で 81 人である。t＝5 の値の目安の標準誤差は 5.5 点になる（本稿の概算）。図の t＝5 の値を、別の表の 10 年生存率 ÷ t＝0 の値と突き合わせると、40 本中 29 本が丸めの範囲に入り、外れた 11 本のはみ出しは最大 2.0 点（肝内胆管がんⅠ期）、それ以外は 0.3 点以内だった（第3節）。

Abstract

This paper asks whether the “5-year survival” of a cancer means the same thing for people who have already lived some years since diagnosis. The answer is no. Survival over “the next 5 years” among those who have lived 1, 2, … years since diagnosis rises more the more advanced the stage, and the gap between stages narrows. And because that shape can be produced either by each person’s body changing or by those who die early leaving the group, aggregate numbers cannot decide between them. No new mathematical theorem and no new law are claimed.

First, the “5-year survival” usually quoted is survival for 5 years from the day of diagnosis. The hospital-based cancer registry tabulation of cases diagnosed in 2012 shows, for the same people, the observed survival over “the next 5 years” among those alive t years after diagnosis (t = 0–5), called survivor survival (conditional survival), for 10 sites × stages I–IV. This paper uses all 40 series (Sections 3 and 4).

Second, this is the core of the paper. At stage IV, for all 10 sites, survival over the next 5 years among those who have lived 5 years (t = 5) exceeds the 5-year survival at diagnosis (t = 0). The median difference is +42.2 points. For pancreatic cancer: stage I 44.0% → 68.7%, stage II 18.3% → 59.0%, stage III 4.7% → 50.1%, stage IV 1.3% → 42.5%; stomach cancer stage IV is 5.5% → 61.2% (Section 5).

Third, the gap between stages (the highest stage minus the lowest) is smaller at t = 5 for all 10 sites. The median gap goes from 55.5 points at t = 0 to 22.4 points at t = 5 (Section 5).

Fourth, 8 of the 40 series are lower at t = 5 (prostate cancer stages I–III −6.6 to −7.1 points, stomach cancer stage I −3.4 points, breast cancer stages I and II −2.4 and −1.8 points, and so on). All 8 are stages I–III. These values are observed survival and include deaths from causes other than cancer (Section 6).

Fifth, the rising shape can be produced by models of two different mechanisms. A “two-group” model in which no one’s risk changes over time (the high-risk group simply leaves first) fits the 32 rising series with a median error of 0.63 points, and a “one-group” model in which everyone’s risk falls over time fits with a median of 1.38 points. Since neither model lets anyone’s risk rise, neither can produce the 8 falling series (Section 7).

Sixth, the numbers have spread. For stage IV pancreatic cancer, those alive at 5 years were 1.3% of the 6,242 people tabulated, about 81 people by approximation. The rough standard error of the t = 5 value is 5.5 points (this paper’s approximation). Checking the t = 5 value of the figure against the 10-year survival of a separate table ÷ the t = 0 value, 29 of the 40 series fall within the rounding range; of the 11 outside, the excess is at most 2.0 points (intrahepatic bile duct cancer stage I), and within 0.3 points for the others (Section 3).
