特定健診（2022 年度）の HbA1c の分布が、年齢とともに平均ごと右へずれるだけなのか、形も変わるのかを、第10回 NDB オープンデータの「HbA1c 都道府県別性年齢階級別分布」（6 つの区分 × 男女 × 40〜74 歳の 5 歳階級）と「各項目の平均値」で確かめた。全国は 47 都道府県と判別不可の人数を足して出した（23,557,356 人）。新しい数学定理も新しい法則も主張しない。

第一に、表はそろっている。中計は 5 歳階級の和に一致し、10 未満で伏せた升目は 36 か所（足せなかった人は多くても 324 人）である（第3節）。

第二に、平均は年齢とともにゆっくり上がる。 40〜44 歳から 70〜74 歳へ、男性 5.485 → 5.911（+0.426）、女性 5.378 → 5.797（+0.419）（第4節）。

第三に、ここが本稿の芯である。線の上の割合の上がり方は、線の高さで違う。 40〜44 歳から 70〜74 歳へ、男性は 5.6 以上 2.16 倍・6.0 以上 4.76 倍・6.5 以上 5.11 倍・8.0 以上 1.77 倍・8.4 以上 1.35 倍で、真ん中の線がいちばん大きく増える（女性も 6.5 以上 7.87 倍がいちばん大きい）（第5節）。

第四に、40〜44 歳の分布を平均のずれだけ右へ動かした見込みと比べると、6.5 以上は男性で見込み 6.67 % に対し実際 16.34 %（2.45 倍）、女性で 3.26 % に対し 9.14 %（2.81 倍）。 男性の 8.4 以上は見込み 1.11 %・実際 1.17 %（1.05 倍）とほぼ見込みどおりで、女性は 1.64 倍（第6節）。

第五に、男性の 8.4 以上の割合は 55〜59 歳（1.58 %）がいちばん高く、70〜74 歳（1.17 %）はそれより低い。 平均は上がり続けるのに、裾の端は途中から増えない（第4節）。

第六に、分けるのは平均がずれたか、分布の形が変わったか。HbA1c では、年齢とともに 6.0〜8.0 のあたりが平均のずれ以上にふくらみ、いちばん上の端はそれほど増えない。

Abstract

This paper checks whether the distribution of HbA1c in the Specific Health Checkups (fiscal 2022) only shifts to the right with its mean as age rises, or also changes shape, using the “HbA1c distribution by prefecture, sex, and age group” (6 bands × men and women × 5-year groups from 40 to 74) and the “mean of each item” of the 10th NDB Open Data. The national figure was obtained by adding the 47 prefectures and the people whose prefecture could not be determined (23,557,356 people). It claims no new mathematical theorem and no new law.

First, the tables are consistent. The subtotals equal the sum of the 5-year groups, and 36 cells are suppressed as below 10 (at most 324 people could not be added) (Section 3).

Second, the mean rises slowly with age. From 40–44 to 70–74, men 5.485 → 5.911 (+0.426), women 5.378 → 5.797 (+0.419) (Section 4).

Third, this is the core of the paper. How fast the share above a line rises depends on the height of the line. From 40–44 to 70–74, for men: 5.6 or above 2.16-fold, 6.0 or above 4.76-fold, 6.5 or above 5.11-fold, 8.0 or above 1.77-fold, 8.4 or above 1.35-fold; the middle line rises most (for women too, 6.5 or above rises most, 7.87-fold) (Section 5).

Fourth, compared with a prediction that shifts the 40–44 distribution to the right by the difference in mean alone, 6.5 or above is predicted at 6.67% but is actually 16.34% for men (2.45 times), and predicted at 3.26% but actually 9.14% for women (2.81 times). For men, 8.4 or above is predicted at 1.11% and is actually 1.17% (1.05 times), almost as predicted; for women it is 1.64 times (Section 6).

Fifth, for men the share at 8.4 or above is highest at 55–59 (1.58%), and lower at 70–74 (1.17%). The mean keeps rising, but the far end of the tail stops increasing partway (Section 4).

Sixth, what separates them is whether the mean shifted or the shape of the distribution changed. For HbA1c, with age the region around 6.0–8.0 swells more than the shift of the mean, and the very top end does not increase as much.
